Advances in Liver Transplantation

With the increasing number of patients waiting for liver transplants and high pre-transplant mortality rates, several advanced techniques have been adopted, including:

Domino Liver Transplantation

This is a sequential transplant where a patient with a suitable metabolic disorder (e.g., familial amyloid polyneuropathy or Maple Syrup Urinary Disease) is grafted with a new liver. The otherwise normal liver is then used for another recipient with end-stage liver disease. This innovative approach has expanded treatment possibilities.

Dual Lobe Grafts

In cases where there is a mismatch between donor and recipient weight, leading to a low graft-to-recipient weight ratio, grafts from two donors can be transplanted into a single recipient. This complex procedure involves three operating rooms and multiple teams working in conjunction. At Sir Ganga Ram Hospital (SGRH), we have successfully performed two Dual Lobe transplants.

Simultaneous Liver-Kidney Transplant

For patients with both end-stage renal and liver disease, a simultaneous liver-kidney transplant can be performed. At SGRH, we have completed eight such transplants using different donors for each organ.

Paired Donor Exchange (Swap)

In cases of donor-recipient incompatibility due to size or blood group mismatch, paired donor exchange can enable both transplants. Although common in kidney transplants, this method is relatively new to living donor liver transplants. We have successfully performed four such paired donor exchanges at SGRH.
